Here are each of the pages you need to complete in order to get cleared for participation during the 2016-17 season:

 

**Team Participation Agreement:  Must be completed for every sport every season

 

**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.

 

Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.

 

**Concussion and Head Injury Information Sheet:  Required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.

 

**Sudden Cardiac Arrest Information Sheet:  Required for all athletes prior to participation, California law 2015.

 

**AT Consent To Treat:  required for athletic trainer to treat athletes

 

**Physical Form:  required every year, must be dated after May 1, 2016

 

**Emergency Form:  Maybe picked up from Coach or in the Athletic Office

Men’s tennis fell to Davis.

 

Varsity softball defeated St. Francis 19-6. Kiannah Pierce had 2 hits. Maci Fines had 3 hits with a double and a triple. Shea Moreno had 2 hits with 2 RBI’s. Sarah Brown hit 2 homeruns and had 3 RBI’s. Taliyah Miles had 2 hits with a homerun and 2 RBI’s. Vianna Barron had 2 RBI’s. Sam Oliver had 2 hits with a double, a homerun, and 3 RBI’s. Taliyah Miles struck out 6 batters.

 

JV softball defeated St. Francis 9-0. Desiree Kayser and Ashley West had 2 hits. Yumiko Hanson hit a homerun.

 

Varsity baseball fell to Jesuit 1-3. Jorge Montoya had 2 hits and Aaron Zeidman hit a triple.

 

JV baseball fell to Jesuit.

WHAT IS AN ALL SPORT PASS?

All - sport passes are for parents and adult family members of our students who wish to attend Sheldon athletic events.  

 

They are on sale now for winter:  $30 for one or a family 4 pack for $90.

 

An All - sport pass will give the holder free admission to all home athletic events with the exception of play-off games.
