Here are each of the pages you need to complete in order to get cleared for participation during the 2016-17 season:

 

**Team Participation Agreement:  Must be completed for every sport every season

 

**Student Alternate Transportation Form:  For students that will either be transporting themselves and/or will be transported by an adult other than a parent/guardian including Coach.

 

Student Personal Auto Usage Form:  For students that will be transporting themselves, requires a copy of current CDL and insurance declaration page in order to be complete.  

Reminder:  if you are filling this form out, you will also need the Student Alternate Transportation Form.

 

**Concussion and Head Injury Information Sheet:  Required for all athletes prior to participation, response to California law that went into effect on Jan 1, 2012.

 

**Sudden Cardiac Arrest Information Sheet:  Required for all athletes prior to participation, California law 2015.

 

**AT Consent To Treat:  required for athletic trainer to treat athletes

 

**Physical Form:  required every year, must be dated after May 1, 2016

 

**Emergency Form:  Maybe picked up from Coach or in the Athletic Office

Men’s volleyball fell to Franklin

 

Men’s golf competed in the Wolfpack Invitational. Jayson Gray shot a team best of 85.

 

Track and Field competed in the Husky Invitational last Saturday:

4 x 100 meter relay quartet of Emily Lemp, Thalia Reid, Tai McDonald, and Tatumn LaChapelle ran just of their season best of 48.15 (State ranked #2), clocking 48.37.

100 meter dash, Section leader Tai McDonald established a new meet record, taking gold in 12.20.

Gold medals were also awarded in the frosh-Soph boy’s 4 x 100 meter relay, with the team of David Norris, Justin Williams, Salik Barber and Dequan Foster clocking 44.85. The Frosh-Soph Boys 4 x 400 meter relay team of Shuntaay VanDyke, Norris, Isaac McInnes, and Justin Williams earned silver in a swift 3:33.32.

Frosh-Soph Boys 200 Meter Dash:      Dequan Foster – 23.95 (4th)

Varsity Girls 3200 Meter Run:             Perla Gonzalez – 12:30.56 (3rd)

Frosh-Soph Boys 800 Meter Run:        Isaac McInnes – 2:07.16 (3rd)

Varsity Girls 100 Meter Hurdles:         Emily Lemp – 16.48 (3rd)

Varsity Boys 1600 Meter Run:            Michael Ruiz – 4:33.15 (5th)

Men’s basketball fell to Woodcreek in the NorCal Championship game. Congratulations to men’s basketball and coach Joey Rollings for a great season! Huskies are proud of you!

 

Varsity baseball defeated Rio Americano 8-2 on Friday and defeated Cordova 14-4 on Saturday.

JV baseball fell to Rio Americano 8-3 on Friday and defeated Cordova 22-4 on Saturday.

All - sport passes are for parents and adult family members of our students who wish to attend Sheldon athletic events.  

 

They are on sale now for winter:  $30 for one or a family 4 pack for $90.

 

An All - sport pass will give the holder free admission to all home athletic events with the exception of play-off games.
